Intervention)
LOCATION: Louisville, KY

REPORTS TO: Program Manager of VOA Health

JOB SUMMARY AND QUALIFICATIONS:
As a partner within Louisville's Pivot Peace Community Violence
Intervention (CVI) Initiative, Volunteers of America Mid-States
will be responsible for providing support for a specific violence
interruption model within four designated program sites that seek
to disrupt and/or mitigate the cycles of gun violence. Community
Health Workers build trusting relationships with high-risk
individuals. Community Health Workers focus on building
relationships with program participants to help connect high-risk
individuals to positive opportunities and resources in the
community, including but not limited to health care, employment,
housing, recreational activities, and education. Community Health
Worker will work with designated violence interrupters and the
Office of Safe and Healthy Neighborhoods to ensure the client
receives the best care possible.
WHAT YOU SHOULD HAVE FOR THIS ROLE:
1. A high school diploma or equivalent is required.
2. Two or more years of experience and a bachelor's degree.
3. Prefer candidates who live within a 10-mile radius of zip codes
40203, 40208, 40210, 40211, 40212, and 40216.
4. Verifiable good driving record and reliable transportation.
5. Background check and fingerprinting.
6. Openness to working closely with clients who have been impacted
by violence, victims of gun violence, and struggle with issues
related to mental illness, substance use disorder, violence, and
chronic health conditions.
7. Respectful sensitivity and non-judgmental attitude towards
clients who may suffer from multiple traumas and disabilities.
8. Strong interpersonal and social skills with an ability to
collaborate with a variety of individuals from a wide range of
professional and personal backgrounds.
9. Ability to convey a strong presence one-on-one, in meetings, and
in the community.
10. Ability to treat everyone equitably.
RESPONSIBILITIES:
1. Develop trusting relationships with clients by providing support
and advocacy to help achieve health goals.
2. Convey the purposes and services of Determined Health to the
member population and the impact of that program or service.
3. Meets members in clinic, facility, office, or at home to help
identify social determinants of health impacting member's health
and general well-being.
4. Follow up with health management/care plans with both members
and providers.
5. Coach members in effectively managing their health conditions
and self-care.
6. Assist members in understanding care plans and instructions.
7. Document activities, service plans, and results in an effective
manner while strictly adhering to the policies and procedures in
place.
8. Conducts and face-to-face outreach in designated areas
9. Help members utilize resources, including scheduling
appointments and assisting with completing applications for
programs for which they may be eligible.
10. Facilitate communication and coordinate services between
providers.
11. Identify and apply appropriate role definitions and skilled
boundaries.
12. Document all activities as detailed by direct supervisors.
13. Navigate clients to community resources to help address social
and economic needs.
14. Approach each client utilizing a cultural humility and health
equity lens.
15. Other duties as assigned.
