• Certification Details
• Current license and primary source verification within 30 days prior to start
• Influenza Vaccine - Needs to be updated every year
• 2 Step PPD OR IGRA (Quantiferon/Tspot) - 1st within 1 year, 2nd within 30 days of start
• Varicella - Documentation of 2 vaccines, immunity evidence or history
• Tdap - Valid for 10 years from the administered date
  • Job Requirements
  • Must be AHA BLS certified
  • Pharmacology 1,2 exam required
  • Ascension Medical Surgical Exam completion
  • Ascension EKG Exam - Score needs to be 90% or higher with only 2 attempts
  • Education needs to be validated at the source level
  • Qualifying reference(s) - At least two professional references
