• Certification Details
• License/Certificate Verification - current license and primary source verification required 30 days prior to start of assignment
• Ascension Criminal Background Check Results required
• Ascension Education Verification must be validated at the source level
  • Job Requirements
  • Must have BLS certification - Must be AHA.
  • Must have ACLS certification - Must be AHA.
  • Pharmacology 1,2 exam required
  • NIH Certification required
  • Ascension EKG Exam - Score needs to be 90% or higher with only 2 attempts.
    • Unit Specific Information
    • Ascension Unit Specific Checklist - Optional for Onboarding, but must be uploaded by the unit leader within a week.
      • Additional Details
      • Vaccines or Positive IGG Titer required; Exemptions are NOT accepted for certain areas.
      • Documentation of Hepatitis B vaccination series completion or declination documentation required
      • Influenza Vaccine required for flu season starting Sept 4th, 2023; no declinations or exemptions accepted.
      • Ascension Onboarding Requirements Education Attestation must be uploaded by the Agency.
      • Ascension Nursing Contract Worker Onboarding to be done onsite.
